Nurse
Iowa Code 139A.8 requires all students enrolling in an Iowa school to submit proof of immunizations to the admitting official using an Iowa Department of Health and Human Services Certificate of Immunization. The District is audited every year in October for immunization compliance. Reminders will begin once school starts if your child is not up to date on their immunizations. Medical or Religious exemptions are accepted.

- Students must have both of these immunizations before starting their 7th grade year. Students with medical or religious exemptions are exempt.
- Dental Screening Form
- Screening must be provided by DDS (Doctor of Dental Surgery), DMD (Doctor of Dental Medicine), or RDH Registered Dental Hygienist. This screening cannot be performed by Primary Care Physicians (MD, DO, PA-C, ARNP).
- Second dose of Meningococcal Vaccination
- or one dose if the first dose was received at 16 years or older
- This must be done before their senior year. Students with medical or religious exemptions are exempt.
Medication Permission Form

- If your student needs to take medication during the school day, please complete, sign, and bring the signed "medication permission form" along with the medication to be administered to the Nurse's Office. The school cannot administer any medication without a consent form by parent/guardian.
- Any pills or medication brought in a baggie or other unlabeled container will not be given at school. Medication must have a current pharmacy label and must be for the student taking the medication (not another family member).
- It is kindly requested and strongly encouraged that a parent or guardian deliver any medication to Nurse Olivia prior to the first day of school. Controlled substances for all ages need to be delivered to Nurse Olivia by a parent/guardian prior to the first day of school.
